There is an interchange with the Sunken Meadow Parkway. In this direction, there is only a northbound entrance. Motorists wishing to go south must use Veterans Highway (NY 454) a few blocks back, although there is no signage to indicate so.

Jericho Turnpike passes under the Long Island Railroad and then ends at a traffic light. Saint Johnland Road (NY 25A) comes in from the left. Straight ahead, NY 25 and NY 25A are multiplexed through Smithtown as Main Street.

Main Street runs east through Smithtown as a four-lane undivided road. There is a railroad overpass, followed by traffic lights at Redwood Lane, New York Avenue, Maple Avenue, Elm Avenue, Lawrence Avenue, Landing Avenue, and Bellemeade Avenue. Signage here is unusually good for the North Shore of Long Island.

Middle Country Road ends here. It widens to become a four-lane divided highway and then splits. Bear left for Old Country Road (CR 58), which begins here. Bear right to stay on NY 25. Immediately after the split, the an exit on the right for the Long Island Expressway (I-495), which also begins here.

The ramp from exit 72 of the Long Island Expressway (I-495) merges in on the right. NY 25 loses its divider and narrows down to two lanes. Here, it is called Main Street.

Turn right onto Sixth Street for access to the ferry to Shelter Island, which leads to the middle section of NY 114.

Third Street (NY 114) begins on the right at a traffic light. Motorists turning right here can only follow NY 114 as far as the ferry and the train station. Those wishing to actually board the ferry to Shelter Island and the middle section must turn off at Sixth Street several blocks back. For this reason, there is no route signage at this intersection.
