Commack Road is a north/south route in western Suffolk. It begins at Deer Park Avenue (NY 231) in North Babylon and runs north to Jericho Turnpike (NY 25) in Commack. It is a two- to four-lane road with traffic lights. Some portions are divided. North of Jericho Turnpike (NY 25), Commack Road changes names to become Town Line Road. It runs north to Northport, where the county route ends at Pulaski Road (CR 11). Town Line Road changes names at this point to become Bread and Cheese Hollow Road, an unnumbered town road that continues north to Fort Salonga Road (NY 25A) in Fort Salonga.
CR 4 was originally proposed to follow the entirety of its present alignment and then continue north along Bread and Cheese Hollow Road. However, until 2010, the county route designation ended a few blocks north of Jericho Turnpike (NY 25). The remainder of Town Line Road and the entirety of Bread and Cheese Hollow Road were among a number of routes known as "county system roads" that were legally designated as county routes but remained under the jurisdiction of the towns. In 2010, the Town of Huntington successfully sued Suffolk County and obtained a court order for the county to take over maintenance responsibility for all of the county system roads within the town. In 2022, the town and county entered into a settlement in which they agreed to a series of maintenance swaps. As part of this, the entirety of Town Line Road became part of CR 4 permanently, while Bread and Cheese Hollow Road was returned to the town. Subsequently, CR 4 signage was erected at the new northern terminus in 2024.
