New York Avenue is formed by the merger of East Shore Road and Youngs Hill Road in Huntington. It runs south as a two-lane undivided road.

There is a pair of roundabouts at Mill Dam Road and Park Avenue, which were part of CR 35 until 2022. The northern portion of the county route has been decommissioned, and they are now unnumbered town road. There is also access to Grist Mill Lane and Creek Road.
