Prospect Avenue begins at Union Avenue (CR 63) in Westbury and runs east as a two-lane road with bike lanes. There is a median in some areas.

The bike lanes end, and Prospect Avenue widens to four lanes for motor vehicles. There is an overpass over the Wantagh Parkway, but there is no interchange here.

Prospect Avenue ends at a traffic light at Cantiague Rock Road. Continue straight ahead onto West John Street to stay on CR 12.

Welcome to Hicksville! West John Street runs east as a four-lane undivided road.

West John Street becomes a divided road. There is a traffic light at Newbridge Road (NY 106). Turn left to go north or right to go south. There is no route signage at this intersection.

West John Street ends at a traffic light at Broadway (NY 107). Turn left to go north or right to go south. To stay on CR 12, turn right here and then immediately left onto East Barclay Street at the next light. East John Street, a local street, is straight ahead. Once again, there is no route signage.

East Barclay Street ends at a traffic light at Bay Avenue. Continue straight ahead onto Woodbury Road to stay on CR 12.

There is a traffic light at South Oyster Bay Road (CR 9). Turn left to go north or right to go south.

Woodbury Road crosses the Northern Parkway, the Seaford-Oyster Bay Expressway (NY 135), and the Long Island Expressway (I-495). There is no interchange with any of the three highways. The expressway has a service road gap here, so it is only possible to get on the service road going west.

CR 12 ends at a traffic light at the border with Suffolk County. Harbor Road (NY 108) begins to the left. Woodbury Road continues straight ahead as Suffolk CR 11.
