The ramps to the Long Island Expressway from the northbound and southbound lanes of the Cross Island Parkway merge together and then split. Bear left to get onto the expressway. Bear right for Douglaston Parkway.

A long ramp leads from exit 38 of the expressway to the Northern Parkway. There is a turnaround that all trucks are required to use, since they are not allowed on the parkway.

The ramp from the eastbound service road to the Sagtikos Parkway south splits twice. Bear right at either split for Crooked Hill Road (CR 13). The ramp from the westbound service road merges from the left between the two splits.

The ramp for exit 68 was constructed to be part of an unbuilt extension of the service road. There is a pair of exits for the William Floyd Parkway (CR 46), followed by an entrance back onto the expressway. The ramp ends ahead at an industrial park.
