NY 27A begins at Sunrise Highway (NY 27) in Massapequa. Immediately, there is a traffic light at Carman Mill Road. Turn right to stay on NY 27A. Old Sunrise Highway is straight ahead.

The section of Merrick Road between Carman Mill Road and the Nassau-Suffolk county line is designated NY 27A. It is locally maintained by Nassau County and also carries the designation of CR 27.

At the Nassau-Suffolk county line, there is an intersection with County Line Road (Suffolk CR 1), which begins here. Nassau CR 27 ends at this point.

There is an interchange with the Southern Parkway. A brown sign marks the eastbound entrance, as the parkway ends one exit east of here at Heckscher State Park. Cars can use the parkway westbound for one exit to reach Sunrise Highway (NY 27), but trucks are prohibited.

Welcome to Great River! There is a traffic light at Connetquot Avenue. Trucks that are prohibited from the parkway can turn left here for access to Sunrise Highway (NY 27).

There is a traffic light at exit 46A of Sunrise Highway (NY 27). There is no entrance to the highway when coming from this direction. NY 27A ends here. Montauk Highway continues straight ahead as CR 85.
