Grand Avenue comes under county maintenance at Atlantic Avenue in Baldwin. It runs north as a two-lane undivided road. There is a stop sign at The Fenway and Grace Street.

There is a traffic light at Merrick Road (CR 27). Turn right to go east or left to go west.

Grand Avenue passes under the Babylon Branch of the Long Island Rail Road at the Baldwin train station.

There is an interchange with the Southern Parkway. Grand Avenue changes names here to become Baldwin Road.

There is a traffic light at Tompkins Place. Baldwin Road splits off to the left to become a southbound one-way street. Henry Street, an unnumbered county road is straight ahead.
