Grand Avenue is a north/south route extending from Baldwin to Hempstead. It begins as a local street at Baldwin Park and runs north for a short distance to Atlantic Avenue, where it comes under county maintenance as CR 55. It runs north past Merrick Road (CR 27) and Sunrise Highway (NY 27) to an interchange with the Southern Parkway just south of the Hempstead village line. There, it changes names to become Baldwin Road and continues north into Hempstead. After a little less than a mile, there is a somewhat confusing arrangement where Baldwin Road splits off to become a one-lane southbound one-way street coming from the junction of Greenwich Street (CR 7B) and Jerusalem Avenue (CR 105) a few blocks to the north, where it ends. The four-lane road that was Baldwin Road becomes Henry Street, an unnumbered county road.
