East Main Road runs west through Middletown as a four-lane undivided road. There is a traffic light at Forest Avenue. The Newport Bridge is visible in the distance.

West of the interchange, Kingstown Road changes names to become Main Street.

After a short distance, Main Street ends at a traffic light Nooseneck Hill Road (RI 3). Continue straight ahead to merge in going south and continue west on RI 138. Turn right to go north.
