Peconic Avenue begins at Main Street (NY 25) in Riverhead. It crosses a bridge over the Peconic River and ends on the other side at a traffic circle at Riverleigh Avenue (CR 104), Flanders Road (NY 24), and Nugent Drive (NY 24/CR 94). CR 63 continues straight ahead as Lake Avenue.
