Old Country Road begins at a grade-separated interchange with Middle Country Road (NY 25) in Riverhead. It has four lanes at first. The right lane ends, and then a lane merges in from exit 73 of the Long Island Expressway (I-495).

Old Country Road runs east as a four-lane undivided road. There are traffic lights at Mill Road, Pulaski Street, Osborn Avenue, and Harrison Avenue.

There is a traffic circle at Roanoke Avenue (CR 73). The signage here is technically wrong. CR 73 has its northern terminus here, and the section of Roanoke Avenue north of here is a local street.

Old Country Road ends at a traffic light NY 25. Continue straight ahead to go east or turn right to go west. Turn left for Doctor's Path.
