Roanoke Avenue becomes a county route coming out of the traffic circle on Old Country Road (CR 58) in Riverhead and runs south as a two-lane undivided road.

There is a pair of traffic lights separated by a railroad crossing. Turn left at the first light for Northville Turnpike (CR 43), which begins here. There is no county route signage at this intersection.

Roanoke Avenue ends at a traffic light at Main Street (NY 25). All vehicles must to right to go west. There is no route signage at this intersection.
