North Jerusalem Road is a short east/west route that splits off from Jerusalem Avenue (CR 105) just east of the Meadowbrook Parkway. While Jerusalem Avenue (CR 105) curves to the south, North Jerusalem Road continues to the east. After a few miles, it ends at Wantagh Avenue (CR 189) in Wantagh. Near its eastern end, it changes names to become Old Jerusalem Road. The entire length is a four-lane undivided road.
