NY 118 begins at Somerstown Road (NY 100) in Yorktown Heights and runs west as a two-lane undivided road. The first section is known as Saw Mill River Road.

There is a traffic light at US 202/NY 35. Turn left onto Crompond Road to go west. Continue straight ahead on Saw Mill River Road to go east and to stay on NY 118.

US 202 splits off onto Lincolndale Road, while NY 118 follows Tomahawk Street, which runs north as a two-lane undivided road.
