CR 23 is a north/south route through the western half of Rockland County. It begins at the border with New Jersey. Orangeburgh Road (Bergen CR 2) becomes Blue Hill Road South, which runs north along the west side of Lake Tappan. After about half a mile, Blue Hill Road South ends at Veterans Memorial Drive (CR 20). The two routes are multiplexed for one block to the east, and then CR 23 turns north again onto Blue Hill Road. After about a mile, Blue Hill Road ends at Convent Road (CR 26). CR 23 turns west onto Sickletown Road. There is another one-block multiplex, and then CR 26 splits off onto Gilbert Avenue. Sickletown Road curves to the north and then turns to the northeast. It crosses the Palisades Parkway with no interchange and then makes its way to West Nyack. There, it has a grade-separated partial interchange with NY 59. One block to the north, it changes names at West Nyack Road to become Strawtown Road.
Strawtown Road crosses the New York Thruway (I-87/I-287), once again with no interchange. It makes its way north along the west side of DeForest Lake. At Congers Road (CR 80), it changes names again to become Ridge Road. It ends at Haverstraw Road (CR 90) in New City. The entire length is a two-lane undivided road.
