CT 181 is a north/south route along the west side of the Barkhamsted Reservoir. It begins at New Hartford Road (US 44) in Barkhamsted. The first section, River Road, runs along the Farmington River until Ripley Hill Road (CT 318). CT 181 is then multiplexed with CT 318 along Pleasant Valley Road across a small bridge over the river. After a short distance, CT 318 splits off to the east onto Saville Dam Road. CT 181 continues north as Center Hill Road to Riverton, where it ends at Riverton Road (CT 20) and Center Street (CT 20).
