PA 423 is a short diagonal route in the Poconos. It begins at PA 940 in Pocono Pines and runs northeast to Tobyhanna, where it has a partial interchange with I-380 and then crosses Pocono Boulevard (PA 611) just south of its northern terminus. It follows Prospect Street, Main Street, and Church Street through Tobyhanna and then continues northeast to South Sterling, where it ends at South Sterling Road (PA 191).
