Woodside Avenue is an east/west route extending from Holtsville to Yaphank. It begins at Patchogue-Holbrook Road (CR 19) just southeast of its interchange with Nicolls Road (CR 97). The westernmost section of Woodside Avenue is a four-lane divided highway. There are a couple of grade-separated interchanges at the beginning, but it quickly becomes a surface road. East of Medford Avenue (NY 112), it narrows down to become a two-lane undivided road. It continues east to Yaphank, where it merges into Horseblock Road (CR 16).
