CR 65 is an east/west route from Sayville to Patchogue. It runs south of Montauk Highway (CR 85), through Bayport and Blue Point. For most of its length, it is known as Middle Road. Near the eastern terminus, Middle Road ends at Atlantic Avenue. CR 65 is then routed north along Atlantic Avenue, east along Weeks Street, north along River Avenue, and east along Division Street to end at the southern terminus of CR 19 in Patchogue.
