CR 51 is a diagonal route extending from East Moriches to Riverhead. It is a four-lane divided highway in most places, although it is undivided south of Sunrise Highway (NY 27). For most of its length, it is known as East Moriches-Riverhead Road. At Lake Avenue (CR 63) in Riverhead, it changes names to become Center Drive South and continues north to end at a traffic circle at Center Drive (NY 24/CR 94).
