It begins at NY 443 and runs east through Altamont to Guilderland, where it follows Western Avenue (US 20) for about half a mile to the west. It splits off onto Carman Road, which runs north toward Schenectady. After crossing the New York Thruway (I-90) just west of exit 25, Carman Road ends at a roundabout at Curry Road (NY 7). NY 146 continues on the other side of the roundabout as Hamburg Street, which runs north into Schenectady. NY 146 follows Altamont Avenue, Duane Avenue, Brandywine Avenue, and Union Street through the city. East of the city line, Union Street continues southeast to end at Troy-Schenectady Road (NY 7) in Niskayuna. However, a few blocks before it ends, NY 146 leaves Union Street and turns onto Balltown Road, which runs north to the Rexford Bridge, where it crosses the Mohawk River. On the other side of the bridge, Balltown Road ends and NY 146 turns east. It passes through Clifton Park, where it has an interchange with the Northway (I-87), and ends at US 4/NY 32 in Mechanicville.
