It begins at Cantitoe Street (NY 22) and runs north through Cross River to North Salem, where it is multiplexed with NY 116 along Titicus Road. It then splits off and continues north into Putnam County. There is a partial interchange with I-84, and then it ends at US 6/US 202. The entire length is a two-lane undivided road.
