NH 12 is a north/south route in the southwestern portion of New Hampshire. School Street (MA 12) becomes NH 12 upon crossing the state line. It runs northwest to NH 101 in Keene and then follows NH 101 west for a short distance to its western terminus at Franklin Pierce Highway (NH 9). NH 12 turns north and is multiplexed with NH 9/NH 10 as a super-2, a two-lane undivided highway with grade-separated interchanges. After a short distance, NH 9/NH 10 splits off to the northeast. NH 12 continues as a super-2 for one more exit before becoming a surface road again. It northwest toward the Connecticut River.

NH 12 meets NH 63 as its northern terminus near the river and turns north, running just east of the river for most of the remainder of its length. In Charlestown, NH 11 joins NH 12, and the two routes are multiplexed to the northeast until Claremont, where NH 11 splits off to the east. After the split, NH 12 runs northwest back to the Connecticut River. This time, it crosses the river to Vermont and becomes VT 12.
