Old Middletown Road begins at Middletown Road (CR 33) in Nanuet and runs south as a two-lane undivided road.

Turn right here onto Convent Road to stay on CR 46. Old Middletown Road continues straight ahead as CR 33A.

Convent Road runs west as a two-lane undivided road. There is a grade crossing with the Pascack Valley Line of the Metro-North Railroad.
